Chronic Care Support (ITC)

The Integrated Team Care (ITC) program is aimed to suupor clients with management of their chronic disease/s. ITC aims to reduce hospital admissions, delay the progression of disease, and assist clients with understanding their medical conditions and ensuring good management of that condition.

The ITC program caters to clients with medical conditions that continue for longer than a 6 month period. ITC workers can help clients book and coordinate doctors appointments, provide transport to and from medical appointments, provide support during appointments and medical procedures, and can help clients manage everyday living with a chronic condition.

To Remain An Active Client In The ITC Program You Must

  • Always inform your ITC coordinator of upcoming appointment/s that require supplementary service funds (otherwise this will affect the payment of your appointment); 
  • Update 715 Health assessment every 9 months (this health assessment can be taken at Waminda);
  • Update GP management plan each year (with 3 month reviews) to remain a current patient/client of Waminda GP clinical services.

Our practice has an appointment booking system for all consultations, we can accommodate walk in consultations with the clinical team, but it is encouraged to make a booked appointment. On arrival you will be triaged by an Aboriginal health worker prior to seeing the Doctor. If you are unable to attend face to face due to symptoms, we have the option of a telehealth appointment.
